5th Annual St. Michael's
Church
Golf Tournament and Dinner
The proceeds from the
tournament to benefit St. Michael's Catholic Church.

Registration and
Buffet Lunch at 11:30 am to 1:00pm at the Club House Patio, Shotgun Start is at 1:30 pm.
We will appreciate golfers be on time to guarantee a successful event. Each team will consists of four players.
The evening will commence with a Gala Dinner at Scoppazi's Restaurant, 13300 Big Basin Way in Boulder Creek. No-host cocktail bar at 5:30pm, dinner at 6:30pm. This will be followed by presentation of prizes, raffle, silent and live action.

Price: $135.00 per player which
includes the following:
Catered Lunch, Snacks, Delicious Dinner
Green Fees, Golf Cart, Golf Goodie Bag, Prizes, and Hole Contests
Awards Presentation
Followed by Dinner at Scopazzi's Restaurant
($40 each for dinner for non-golfers)
Dinner table assignments will be groups of 4 to 10 people. We will
try to have the foursomes with their guests at the same table.
Dinner Choices: (#1) Beef, (#2) Grilled Salmon or (#3) Vegetarian
Above dinner entrees include fresh garden salad,
potato (rice w/seafood), fresh vegetable,
French bread, dessert, coffee or tea, glass of House Wine, tax & tip. |

Hole Sponsors will have their name and/or their business advertised on a T sign at the hole.
All Sponsors and other Donors of cash, goods or services, will be acknowledged in a brochure

Each year, St. Michael's
is faced with the task of raising funds to cover on-going
church expenses. With this year's event, 100% of the proceeds will
benefit St. Michael's Catholic Church.
